input/output error with MegaRaid LSI 9271

I have XenServer 7 on SuperMicro , 2 SSD disks and 2 SATA disks on LSI Megaraid 92714i
Yesterday I noticed my server was frozen with no access via xsconsole, I login via SSH and type:
du -h , I got input/output error
ifconfig , I got input/output error

I rebooted the system and its working now, but I dont want this occur again.

lspci | grep -i megaraid
02:00.0 RAID bus controller: LSI Logic / Symbios Logic MegaRAID SAS 2208 [Thunderbolt] (rev 05)

I dont have 3party drivers, full "original" xen7 with all updates
Some directions about this?
